A Brief Overview
Hospitalist Case Managers are a direct extension of the Hospitalist providers who care for adult patients 18 yrs. of age and older. Within the nursing profession, they utilize their skills and expertise to proactively collaborate with the interdisciplinary team to discuss patient/family needs for necessary services within the continuum of care and to prepare for the transition of care at discharge. Hospitalist Case Managers assume responsibility and accountability for a panel of patients in collaboration with a Hospitalist during the patient's hospitalization. Hospitalist Case Managers communicate directly with Hospitalist providers regarding their patient assignment and carry out tasks and duties related to the patient's individual plan of care. Hospitalist Case Managers are responsible for preparing a summary of care for the patient's hospitalization which is provided to the physician for final signature at time of discharge.

Education Qualifications
- Bachelor's of Science in Nursing (BSN) Preferred
- 5 years Acute care, broad based nursing experience in a hospital setting. Preferred
- Quality management/Joint Commission, discharge planning and/or case management experience is equivalent to acute care nursing experience Preferred
- Experience conducting clinical assessments, setting up home care services, general discharge planning and/or case management . Preferred
- Demonstrates critical thinking skills through problem solving and decision-making. (Required proficiency)
- Time management, verbal and written communication skills, listening skills, prioritizing, and work organization. (Required proficiency)
- Knowledge of professional nursing theory and practice to give and evaluate patient care. (Required proficiency)
- Knowledge of organizational policies, regulations, and procedures to administer patient care. (Required proficiency)
- Knowledge of common safety hazards and precautions to establish a safe work environment. (Required proficiency)
- Maintains a professional level of conduct, productivity, and coordinates and prioritizes patient related activities in an efficient and effective manner. (Required proficiency)
- Registered Nurse - KSBN A multistate license with the ability to practice within the State of Kansas is also accepted. Required
- Basic Life Support - BLS Required within 90 days of hire.
- Facilitates coordination of care and discharge planning in conjunction with the health care team for patients transitioning to the next level of care.
- Communicates with all members of the health care team informing them of any concerns or recommendations regarding discharge needs or plans.
- Maintain relationships with community partners across the continuum of care.
- Keeps supervisor informed of all quality and risk issues discovered through appropriate channels.
- Identifies areas for process improvement and participates in related activities.
- Identifies barriers to achieving goals in the plan of care and uses critical thinking skills to resolve.
- Professionally presents self to patient/family as a representative of the Hospitalist team.
- Educate and facilitate critical thinking with primary nursing regarding acute patient medical management.
- Assures completion of discharge paperwork including, but not limited to, discharge summary, FMLA, return to work statements, prior authorizations, attending physician forms, and discharge referrals.
- Coordinates post-discharge follow-up care with accepting facility, primary care or sub-specialty office.
- Participates in hospital committees and special projects to improve team or hospital performance.
- Appropriately delegates tasks and duties in the direction and coordination of health care team members, patient care and department activities in accordance with the Kansas State Nurses Practice Act.
- Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
